If your refund is fully taken you are responsible for paying the preparation fees. 1040 MTG provides the taxpayer with ONE complimentary copy of their tax return. Should you need any additional copies, there is a $25 fee per copy (federal and state included). 1040 MTG is not responsible for any discussions or changes the IRS or bank may make on disbursement dates, filing status or any other required information from the IRS.1040 MTG is not responsible for any IRS glitch problems or IRS problems that may cause a delay in your tax refund. We DO NOT reimburse any bank fees in the event of this occurrence.

Consent To Use

Federal law requires this consent form be provided to you ("you" refers to each taxpayer, if more than one). Unless authorized by law, we cannot use your tax return information for purposes other than the preparation and filing of your tax return without your consent. You are not required to complete this form to engage our tax return preparation services. If we obtain your signature on this form by conditioning our tax return preparation services on your consent, your consent will not be valid. Your consent is valid for the amount of time that you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature. For your convenience, we have entered into an arrangement with certain companies to offer an Electronic Refund Disbursement Service and/or Loan product. To determine whether these offerings may be of interest to you, we will need to use your tax return information. If you would like us to use your tax return information to determine whether these offerings are relevant to you while we are preparing your return, please sign and date this consent to the use of your tax return information. By signing below, you authorize us to use the information you provide to us during the preparation of your 2025 tax return to determine whether to offer you an opportunity to apply for the Electronic Refund Disbursement

Service and/or Loan product.

If you consent to the disclosure of your tax return information, Federal law may not protect your tax return information from further use or distribution. You are not required to complete this form to engage our tax return preparation services. If we obtain your signature on this form by conditioning our tax return preparation services on your consent, your consent will not be valid. If you agree to the disclosure of your tax return information, your consent is valid for the amount of time that you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature. You have indicated that you are interested in utilizing the Electronic Refund Disbursement Service and/or Loan product from Republic Bank or Santa Barbara Trust. To provide you with the opportunity to apply for and/ or receive the Electronic Refund Disbursement Service and/or Loan product, we must disclose all of your 2025 tax return information to Santa Barbara TPG bank or Refund Advantage. You may request a more limited disclosure of tax return information, but you will not be eligible to submit an application for the Electronic Refund Disbursement Service and/or Loan product. If you would like us to disclose your 2025 tax return information to Refund Advantage or Santa Barbara TGP for this purpose, please sign and date your consent to the disclosure of your tax return information. By signing below, you authorize us to disclose to the designated bank all of your 2025 tax return information so that it can evaluate and process your application for the Electronic Refund Disbursement Service and/or Loan product. You understand that if you are not willing to authorize us to share your tax information with our designated banks, you will not be able to obtain the Electronic Refund Disbursement Service and/or Loan product, but you can still choose to have your tax return prepared and filed by us for a fee. We here at 1040 MTG value the relationship we have with all of our clients. We are in the business to provide you with the absolute best service you can have! With that being said, we would like to remind you that we are providing you a service, not a refund! You as a taxpayer have the choice to pay your tax preparation fees by one of the following: cash, check, money order, credit card, or directly from your tax refund. If you choose to have the tax preparation fees taken from your refund, there are additional banking fees aside from your original tax preparation fee. If you opt in to have a cash advance, there are also additional fees in addition from your original tax preparation fee, plus the banking fees, depending on the amount of the cash advance (which is up to $7500). A copy of the fee disclosures will be given to you prior to opting in for anything.

*Please also note, that if you choose to have the tax preparation fees taken from your refund and for whatever reason your taxes are intercepted, YOU ARE STILL RESPONSIBLE FOR THE TAX PREPARATION FEES!  Please contact us immediately at our office for payment or payment arrangements. By signing your return, you are agreeing to pay the preparation fees regardless of if your refund was received or not. After 90 days from the date the refund was scheduled to arrive, if no payment or payment arrangement is made, we will be forced to put your account into collections and/or debit your bank account. We hope we never have to go this route, so please keep open communication with us so we can work with you!
